ATTESTATION FORM
Daily attestations
A daily health check, or attestation, is one of the ways Lake Stevens School District is working to minimize the spread of COVID-19 in our district. Families are required to complete an electronic attestation every day each child who attends school in person. The attestation asks questions about health symptoms, exposure history, and testing related to COVID-19. Each elementary school has its own unique attestation link on its website.
This link is for Hillcrest Elementary.
