Immunization Requirements

1. Measles, Mumps, Rubella (MMR) Vaccine

  • In May 2019, the Washington State Legislature passed a bill that removes the personal and philosophical option to exempt children from the MMR vaccine required for school entry.  Click here for the press release.

3. Polio Requirement

  • K-5th grade: the final dose of IPV, given on or after August 7, 2009, must be given on or after 4 years of age AND a minimum interval of 6 months from the previous dose.

4. Hepatitis B Requirement

  • K-9th grade: Dose 3 must be given on or after 24 weeks of age; minimum interval between dose 1 and dose 3 is 16 weeks.
  • 10th-12th grade: Dose 3 must be given on or after 4 months of age; minimum interval between dose 1 and dose 3 is 12 weeks.

As of August 1, 2020, the revised rule requires medically verified immunization records for school and child care entry.

Washington State Law allows parents or guardians to exempt their child from the school or childcare immunization requirements. Exemptions may be claimed for personal/philosophical, religious or medical reasons.

 Washington State's Immunization Registry
Washington State's Immunization Registry Lake Stevens School District Registered Nurses and a limited number of approved staff working with the school nurse utilize Washington State Immunization Information System (WIIS) to access student's immunization histories which have been provided by the child's healthcare provider. All district employees with access to this registry have signed a confidentiality agreement and operate under the guidelines set forth by the HIPAA Privacy rule and FERPA Act.

Access to this registry allows the school nurse to update student immunization records in an accurate and timely manner, greatly reducing the number of phone calls and mailings to parents requesting this necessary documentation which is required for school attendance.

On the student health history form, there is a section for you to give your permission for your student's immunization information obtained through WIIS to be transferred to your student's CIS (Certification of Immunization Status) form which is a part of the student's educational record.

You can now access this information for your family. By accessing your family's immunization information you can view, download and print the completed CIS. Once you have printed and signed the CIS it is now ready to be turned into your child's school for enrollment.
