Immunization Requirements

1. Requirements for Haemophilus Influenza type b (HIB) Vaccine

  • Preschool/Transitional K: 3 or 4 doses (depending on vaccine), not required at 5 years of age or older

2. Requirements for Pneumococcal (PCV) Vaccine

  • Preschool/Transitional K: 4 doses; doses may be acceptable with fewer than listed depending on when they were given

3. Requirements for Measles, Mumps, Rubella (MMR) Vaccine

In May 2019, the Washington State Legislature passed a bill that removes the personal and philosophical option to exempt children from the MMR vaccine required for school entry.

  • K-12th grade: 2 doses

4. Requirements for Varicella (Chicken Pox) Vaccine

  • Preschool: 1 dose
  • K-12th grade: 2 doses

5. Requirements for Polio Vaccine

  • K-12th grade: 4 doses; doses may be acceptable with fewer than listed depending on when they were given

6. Requirements for Hepatitis B Vaccine

K-12th grade: 3 doses

7. Requirements for Diphtheria, Tetanus, Pertussis (Whooping Cough) DTap/Tdap

  • Preschool: 4 doses DTap
  • Preschool/Transitional K: 5 doses DTap
  • K-6th grade: 5 doses DTap
  • 7th-11th grade: 5 doses DTap plus Tdap at age 10 years and older
  • 12th grade: 5 doses DTap plus Tdap at age 7 years and older

As of August 1, 2020, the revised rule requires medically verified immunization records for school and child care entry.

Certificate of Exemption

Washington State Law allows parents or guardians to exempt their child from the school or childcare immunization requirements. Exemptions may be claimed for personal/philosophical, religious or medical reasons.

Washington State Immunization Information System (IIS) School Immunization Module (School Module)

The School Module is a portal into the Washington State Immunization Information System, or WAIIS, that schools use for immunization tracking and reporting. It can be used to access students' existing immunization records. It allows users to see school specific information and reports. The goal of this system is to improve children's compliance rates and to keep children healthy and protected from vaccine-preventable diseases.

The Lake Stevens School District Registered Nurses and a limited number of approved staff working with the school nurse utilize the School Module to access students' immunization histories, which have been provided by the child's healthcare provider.  All district employees with access to the School Module have signed a confidentiality agreement and operate under the guidelines set forth by the HIPAA Privacy rule and FERPA Act.

On the Certificate of Immunization Status (CIS) form, there is a section for you to sign and give the school permission to add your student's immunization information into the IIS.  Giving your permission will assist the school in maintaining your student's immunization record in an accurate and timely manner.
